• Barbara's Five Page Assessment Form (Fill Out What You Can, We Will Fill Out the Rest Together)
• Face Sheet (General Information-Fill Out What Is Pertinent For You)
• If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.),  or family member, complete this form to authorize release of psychotherapy information:
